MINPINERIE DOG RESCUE FOSTER APPLICATION

Please note: As a foster parent, your current pet MUST be spayed/ neutered and fup to date on all vaccines. (Unless excepted for medical reasons, age, etc. This would require veterinary confirmation)

To avoid a delay in processing your application to foster a rescued Minpinerie Dog Rescue dog, please answer all the questions. Once completed, please email the application to loulouonline@rogers.com.

You can send as an attached word document, scanned PDF, or even send a photo of the completed document which you have printed and filled out by hand. Please ensure that the document is legible.

The information requested will help us, when possible,to match you to a pup that might better meet your (and the dog’s) needs. It is also possible that you maybe be asked to foster a dog about whom very little is known when it comes into rescue i.e. a pup found as a stray, or abandoned at a shelter.

We will make every effort to contact all applicants within 48 hours of receiving their application. This will usually be done by telephone. Please include best times to call you if there are times when you are not available, (i.e job, evenings)

NOTE: MINPINERIE ONLY RESCUES DOGS UNDER 20LBS AND NOT ALL DOGS REQUIRE THE OWNER TO HAVE A YARD. Many small dogs make great apartment companions, others may need easier and more frequent access to the outside or actually need a private play place for extra exercise.

NOTE
IMPORTANT: Each of the following Minpinerie Foster requirements must be adhered to at all times, without exception:

• All Minpinerie dog placements & adoptions must be through the Minpinerie Application/Adoption process with approval by the Adoption Co-ordinator
• All Minpinerie fostered dogs belong to Minpinerie Dog Rescue and must be returned to Minpinerie Dog Rescue if requested to do so.
• Fostered Minpinerie Rescue dogs must reside at the foster family’s address) stated on this Application and may not be given to anyone else or placed in any other home or facility (unless such temporary placement is agreed to in advance with the foster coordinator.)
• If the Foster family can no longer care for the fostered Minpinerie Rescue dog, the foster must immediately contact the Minpinerie Dog Rescue Foster and Adoption Coordinator.
• The fostered Minpinerie Rescue dog may not be sold, abandoned, relinquished, transferred or given to a shelter or another rescue group, person, persons, group, organization or institution.
